How to Create a Maintenance Log: 6 Fields to Track

Every maintenance log starts the same way: someone writes down what they fixed. Every maintenance log fails the same way too. Six months later nobody can tell whether the thing was serviced on schedule. Or who signed off. Or which of the four identical machines it was.

The difference between a notebook and a usable record is which fields you capture, consistently, every time.

This guide sets out six fields that make a log defensible and shows what one regulator requires in writing. It also covers retention periods and walks through building the log from work orders and invoices you already have.

What a maintenance log is

A maintenance log is a running record of work performed on an asset — what was inspected, what was repaired, when, and by whom.

It is distinct from two documents it often gets confused with. A maintenance schedule is forward-looking: what is due and when. A maintenance checklist is procedural: the steps a technician follows during one service. The log is the history — the evidence that the schedule was followed and the checklist was completed.

That evidentiary role is why the log outlives the equipment. It supports warranty claims, resale value, insurance questions, safety investigations and regulatory inspections. All of those are situations where someone else decides whether your record is good enough.

The 6 fields every maintenance log needs

These six columns cover what an outside reader needs to accept the record. The first four map directly onto what one US regulator requires in writing, which the next section quotes.

1. Asset identification. Not "forklift" but the specific unit — an internal number, the make, the serial number, and the year. Where several identical assets exist, the serial number is the only field that reliably separates them.

2. Ownership or source. Who the asset belongs to, and if it is leased or supplied by someone else, who furnished it. This field seems redundant until an asset changes hands mid-life and the history has to travel with it.

3. Next-due indicator. What service is coming and when it falls due. A log without this is only a history; with it, the same document answers "what needs doing this week?"

4. Work performed, with date and nature. The date the work happened and what the work was. Both parts matter — a date with no description proves nothing, and a description with no date proves it happened sometime.

5. Parts, materials and cost. Which parts went in and what they cost. This is the field that turns a compliance record into a budgeting tool. Repeated part replacement on one asset is the signal that it is nearing end of life.

6. Technician and verification. Who performed the work, and who confirmed it was completed. A named person converts an assertion into an attributable one, which is the whole point when a record is read years later.

What the regulations actually require

Most industries have no legally prescribed log format, which is why people invent one. It helps to look at a sector where the fields are written into law. The structure transfers even when the rule does not apply to you.

In the United States, commercial motor carriers fall under 49 CFR Part 396, "Inspection, Repair, and Maintenance." The regulation requires carriers to keep records for each vehicle they control, and specifies their contents.

The record must include "an identification of the vehicle including company number, if so marked, make, serial number, and year." Where the vehicle is not owned by the carrier, "the record shall identify the name of the person furnishing the vehicle."

It must also include "a means to indicate the nature and due date of the various inspection and maintenance operations to be performed." And it must carry "a record of inspection, repairs, and maintenance indicating their date and nature."

Read those three requirements together and you have fields one through four above. The regulator is not asking for a narrative. It is asking for identity, ownership, what is due, and what was done.

The rule also permits digital records. Part 396 states that a driver vehicle inspection report "may be created and maintained in electronic format." A spreadsheet or a database therefore satisfies the form requirement where a paper book once did.

To be clear about scope: this regulation governs commercial motor carriers. It does not apply to a factory's machine tools or a building's HVAC plant. It is cited here because it is a rare case of a regulator stating the fields explicitly. Those fields are also the ones an insurer or auditor tends to look for regardless of sector.

How long you have to keep the records

Retention is the part teams get wrong most often. They either keep everything forever in a way that makes nothing findable, or clear out records at the wrong moment.

Part 396 gives two different periods for two different documents, which is instructive even outside trucking.

Maintenance records must be "retained where the vehicle is either housed or maintained for a period of 1 year." They must also be kept "for 6 months after the motor vehicle leaves the motor carrier's control." Note that second clause: the obligation continues after the asset is gone.

Driver vehicle inspection reports have a shorter clock. The carrier keeps the report, the certification of repairs, and the certification of the driver's review. The period is "three months from the date the written report was prepared."

Two lessons generalize. Retention should be tied to the asset's lifecycle rather than to a calendar year. And disposal of an asset is not the moment to delete its history.

What you need before you start

Gather the inputs before building the table, because a maintenance log assembled from memory has the same weakness as any reconstructed record.

Work orders or service tickets for the period. Invoices from external service providers, which carry the dates and parts. The manufacturer's recommended service intervals for each asset, which populate the next-due field. Your asset register, or whatever list exists of serial numbers. And any existing log, however partial.

If those live in four places, that is normal — and it is the reason the manual version is slow.

How to create a maintenance log with AI

The tedious part is not the table. It is reading forty invoices and service tickets and pulling the same six facts out of each, where every vendor formats theirs differently.

Step 1: Upload the work orders, invoices and asset list

Putting the invoices and the asset register in the same workspace is what lets the serial numbers be matched up. An invoice that says "forklift repair" becomes useful only when it can be tied to a specific unit. The register is what makes that link possible.

Step 2: Describe the log and name the six fields

Ask for the table in natural language and list the columns. Those are asset identifier with make and serial, owner or supplier, and next service due. Then work performed with date and nature, parts and cost, and technician with verification.

Add two instructions worth their weight. Ask for any invoice that cannot be matched to a known asset to be flagged rather than guessed at. An unmatched service record attached to the wrong machine is worse than one left out. And ask for the next-due dates to be calculated from the last service date plus the stated interval, so the schedule column populates itself.

Step 3: Resolve the unmatched records and export

Work through the flagged rows first. An invoice that will not match usually means a serial number was mistyped on one side. Or the asset is not in the register at all, and a missing asset is a finding in itself.

Check that every asset in the register appears at least once. An asset with no maintenance history is either new, or it has been missed, and knowing which is the reason to run this exercise.

Export to Excel and keep the source invoices in a folder alongside it. Each entry traces back to the document it came from. That is what makes the log hold up when read by someone who was not there.

How to build one in a spreadsheet

For a small fleet or a handful of machines, a spreadsheet is a perfectly good system.

Use one sheet per asset class rather than one per asset. Splitting by asset means fifty tabs nobody maintains; splitting by class keeps like-for-like comparison possible and the number of tabs manageable.

Put the six fields in the order listed above, left to right. Identity and ownership are the columns you sort and filter by, so they belong at the left where they stay visible while scrolling.

Make next-due a formula, not a typed date. Last service date plus interval keeps itself current, and a typed date goes stale the moment someone forgets to edit it.

Add a conditional format that highlights rows where the next-due date is within thirty days. That single rule converts the log from a record into a work list. A work list is the thing people actually keep up to date.

Never overwrite a row. Maintenance is append-only: a new service is a new row, not an edit to the last one. Editing in place destroys the history you built the log to preserve.

Maintenance log vs schedule vs checklist vs register

Four terms circulate and they are not interchangeable. Getting them straight avoids building the wrong document.

A maintenance log is the history of work done. A maintenance schedule is the plan of work due. A maintenance checklist is the procedure for a single service. A maintenance register — sometimes called an asset register — is the list of what you own.

They chain together. The register says what exists and the schedule says when each item needs attention. The checklist says what to do, and the log records that it was done. Teams that try to run all four from one sheet usually end up with a document that serves none of the purposes well.

The exception is the next-due field in the log, which deliberately borrows from the schedule. Keeping that one column in the history is what makes the log answer forward-looking questions without maintaining two documents in parallel.

Common mistakes

Recording the asset type instead of the asset. "Compressor" is not an identifier. Serial numbers exist because identical machines wear differently.

Skipping the "no fault found" visits. An inspection that found nothing is still evidence the inspection happened. It is exactly the record you want when someone asks whether the schedule was followed.

Editing rows instead of appending them. Overwriting the last service entry erases the interval between services, which is the most useful thing in the log.

Letting next-due dates be typed. They go stale silently. A formula does not.

Deleting records when the asset is sold. Part 396 requires six months of retention after a vehicle leaves the carrier's control. The general principle holds broadly, because warranty and liability questions outlast ownership.
